Joints, bones and mobility: cartilage bioregulators, collagen and what the evidence shows
For stiff or aching knees and hips, exercise has the strongest evidence. A Cochrane review of 44 trials found that land-based exercise reduced knee pain, and the 2019 American College of Rheumatology guideline strongly recommends exercise and weight loss. For bone, supervised heavy resistance and impact training raised spine bone density in postmenopausal women with low bone mass. Cartilage and collagen bioregulator capsules have almost no human data: one small uncontrolled Russian report on Sigumir. Treat them as optional extras.

Human studies: what works for joints and bones
Movement is the treatment with the most evidence. A 2015 Cochrane review of land-based exercise for knee osteoarthritis found high-quality evidence that it reduced pain (about 12 points on a 0–100 scale) and moderate-quality evidence that it improved physical function. The 2019 ACR/Arthritis Foundation guideline strongly recommends exercise, weight loss for people who are overweight, tai chi and self-management programs.
For bone, the LIFTMOR trial randomized 101 postmenopausal women with low bone mass to 8 months of supervised high-intensity resistance and impact training or home-based low-intensity exercise. The training group gained lumbar spine bone density (+2.9% vs −1.2%) and did better on every function test, with one minor adverse event. Start such training only with supervision.
Collagen supplements are a different ingredient from collagen bioregulators. In one randomized trial, 40 mg a day of undenatured type II collagen (UC-II) improved knee symptom scores versus placebo over 180 days. That says nothing about a 15 mg peptide-fraction capsule.

Human studies: cartilage bioregulators
The only human report we found is a 2012 Russian study of Sigumir (the GARMONIA cartilage-and-bone Cytomax) given alongside dental prosthetics, physical therapy and medicines to 62 older adults with jaw-joint problems. There was no control group, so the effect of Sigumir cannot be separated from the rest of the care.
No human trial has tested Cartesyl, Cartalax, Collacell or any other cartilage or collagen bioregulator capsule.
Lab studies and reviews
A 2023 Russian narrative review discusses aging chondrocytes and proposes peptide bioregulation as a direction for research. It is a review of hypotheses, not a trial.
How to choose
If joints ache, start with a structured exercise program (a physical therapist can tailor one) and, if relevant, weight loss. Those have outcome data; capsules do not.
If you still want to try a bioregulator, Cartesyl (cartilage) and Collacell (collagen) are the Vita house-line options, and Sigumir and Cartalax are the original GARMONIA and Cytogen products. One product for one month, a symptom diary, then decide. Bonocore is a bone-marrow product, not a bone-density product.

Do cartilage bioregulators rebuild cartilage?
There is no evidence of that in people. The only human report is one small uncontrolled study of Sigumir given alongside other treatment. No capsule has been tested in a controlled trial.
Is Collacell the same as a collagen supplement?
No. Collagen supplements are grams of hydrolyzed collagen or about 40 mg of undenatured type II collagen. Collacell is a 15 mg collagen peptide fraction sold as a bioregulator. The collagen-supplement trials do not apply to it.
What helps knee pain most according to research?
Land-based therapeutic exercise (high-quality Cochrane evidence) and, for people who are overweight, weight loss. The ACR 2019 guideline strongly recommends both.
Can bioregulators improve bone density?
No bioregulator has been shown to change bone density. Supervised resistance and impact training raised spine bone density in the LIFTMOR trial, and prescription medicines exist for osteoporosis. Ask your clinician about a bone density scan if you are at risk.
