Creatine for brain health and memory: what the research shows

Creatine is best known for muscle, but the brain also uses it to recycle energy. Pooled randomized trials suggest a small improvement in memory overall, with a larger effect in adults aged 66 to 76 and essentially none in younger people. Results for attention, reaction time and other skills are conflicting. A lab study found that a single high dose improved thinking speed during a night without sleep. Creatine is among the best-studied supplements for safety in healthy people, but it has not been shown to prevent cognitive decline.

Why creatine might matter for the brain

Creatine helps cells regenerate ATP, their short-term energy currency. The brain is one of the most energy-hungry organs, so researchers have asked whether topping up creatine stores helps thinking, especially when energy demand is high or when stores may be lower, such as in older adults and vegetarians.

What the randomized trials show

A 2023 meta-analysis pooled 8 placebo-controlled trials on memory in healthy people. Creatine improved memory measures overall by a small amount (standardized mean difference 0.29). The effect was larger in older adults aged 66 to 76 (0.88) and absent in people aged 11 to 31 (0.03). The trials varied a lot (heterogeneity was high), and dose, duration and sex did not change the result.

An earlier systematic review of 6 trials (281 people) found that short-term memory and reasoning may improve, while results for long-term memory, attention, executive function, reaction time and mental fatigue were conflicting. Young people's performance did not change, and vegetarians responded better on memory tasks than meat-eaters.

Creatine and sleep loss

Brain creatine usually rises only after weeks of supplementation. A 2024 lab study tested a single high dose (0.35 g per kg of body weight) during about 21 hours without sleep and measured brain energy compounds with magnetic resonance spectroscopy. Creatine improved cognitive performance and processing speed and changed brain energy markers compared with placebo. It was a short, controlled lab study, not a test of everyday use.

Safety

The International Society of Sports Nutrition's position stand concludes that short- and long-term creatine supplementation (up to 30 g a day for 5 years) is safe and well tolerated in healthy individuals and in a number of patient groups. People with kidney disease or who take medicines that affect the kidneys should talk to a clinician first.

A different approach: brain peptide bioregulators (Vita-Stream)
