NMN vs NR vs NAD+ injections: what the human trials actually show

NMN and NR both reliably raise NAD+ in the blood of people who take them. A randomized comparison published in 2026 found that 14 days of either raised circulating NAD+ by a similar amount, while plain nicotinamide did not. What is not settled is whether that rise improves health: a 2026 systematic review of 33 human studies found effects on function, metabolism and blood vessels that were mixed and often null. No outcome trial has tested injected or intravenous NAD+ for healthy aging. Neither pill nor shot has been shown to slow aging in people.

What NMN, NR and NAD+ are

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme every cell uses to turn food into energy and to run repair enzymes. Animal studies link falling NAD+ to aging, which is why "NAD+ boosting" became a longevity trend.

NMN (nicotinamide mononucleotide) and NR (nicotinamide riboside) are forms of vitamin B3 that the body can turn into NAD+. Nicotinamide is the cheapest B3 form. NAD+ itself is also sold as injections and IV drips through clinics.

Do they raise NAD+? Yes, in blood

NR: in a randomized, double-blind crossover trial in healthy middle-aged and older adults (two 6-week periods), NR was well tolerated and raised NAD+ metabolism. NMN: in healthy older men, 250 mg a day for 6 or 12 weeks raised whole-blood NAD+ and its metabolites, with no harmful effects found.

Dose and timing: in a 60-day trial of 80 healthy middle-aged adults, 300, 600 and 900 mg of NMN all raised blood NAD+, highest at 600 and 900 mg. Three authors were employees of Abinopharm or Aba Chemicals. In a small 2026 pharmacokinetic study of NR or NMN (12 people, half with Parkinson's disease), blood NAD+ rose slowly and plateaued after about two weeks; brain NAD+ rose measurably after four weeks.

Head to head: a 2026 randomized, open-label, placebo-controlled study in 65 healthy adults found that 14 days of NR or NMN raised circulating NAD+ by a similar amount, while nicotinamide did not. In lab fermentation with human gut bacteria, NR and NMN were converted to nicotinic acid, which the authors propose explains part of the effect.

Do they improve health? Mixed results

Some trials found a measurable change. In a 10-week trial in postmenopausal women with prediabetes and overweight or obesity, NMN increased muscle insulin sensitivity measured by the clamp method. In 12 older men, NR for 21 days raised the muscle NAD+ metabolome and lowered circulating inflammatory cytokines, without changing mitochondrial energy output.

Others did not. In 40 men with obesity and insulin resistance, NR at 2,000 mg a day for 12 weeks did not improve insulin sensitivity, glucose metabolism or body composition. A 2026 systematic review of 33 human intervention studies concluded that NR and NMN clearly engage their target but that effects on functional, metabolic and vascular outcomes are heterogeneous and often null.

NAD+ injections and IV drips

Injected NAD+ skips digestion, which is the selling point. But the same 2026 systematic review found no eligible outcome trials of intravenous or intramuscular NAD+ for anti-aging or wellness; it found only a pharmacokinetic pilot. Injectable NAD+ is a compounded prescription product, not an FDA-approved drug for this use.

Legal status and safety

In 2022 the FDA took the position that NMN could not be sold as a dietary supplement. On September 29, 2025 it reversed that view after a citizen petition, and it has since told companies that NMN is not excluded from the dietary supplement definition. That does not mean every product has been reviewed: the FDA's December 2025 letter reinstated its earlier objections to two specific new-dietary-ingredient notifications for NMN, and each product still has to meet the normal rules for supplements.

In the trials above, NR and NMN were generally well tolerated over weeks to months. Long-term safety data are lacking. Talk to a clinician first if you take medicines or have a chronic condition.
